How many boxes will you need (up to 5 people per box) Required

Please list names of all persons that will be tending a garden plot and one form of contact (email or phone number). Please also indicate if they prefer text or phone call. If the member needs to be contacted through their care provider please add "contact care provider" after their name. (example: James Murray, 503-693-4920, text)

Please add names and contact (phone or email) of all providers that will be supporting members at the garden. Please add the name of the person they are supporting. (example: Cloe Green, Cloegreen@gmail.com, supports James Murray)

Check this box if you need a garden plot on the sidewalk for the use of people in wheelchairs or limited mobility.

Aspire Together uses pictures of members in the garden to promote the program on social media, in newsletters, and while seeking funding. Checking this box indicates that your agency understands and members listed above agree to allow us to use their pictures unless otherwise indicated in the box below.

List any garden members or staff here that DO NOT allow pictures on social media or other forms of program promotion. (Aspire Together takes the privacy of our garden members very seriously will not post pictures if members do not want them used)

Our agencies staff will do their best to help members take care of their garden beds by attending to the garden at least once a week.

Our staff will help members show respect and show kindness their fellow gardeners.

Our staff will encourage members to respect nature and the garden by picking up garbage, pulling weeds, and taking care of the space.

Our staff will encourage members to attend the garden classes and participate in the program activities. They will inform members about the activities and keep Aspire Together informed about whether or not they will be in attendence.

Our agency understands that the church is letting us use their property and we will help our members respect the space and encourage them to take responsibility for any damages. (Please do not use the playground, Aspire Together is not liable for playground related injuries)

Your Signature

Once submitted you will receive an email about acceptance into our program. We ask that each gardener pays a small fee of $15 to help us with the upkeep of the garden. Scholarships are available for those unable to pay this fee.
